The focus of hospice care relies on the belief that each of us has the right to pass pain-free and with dignity. It is also important to know that our loved ones will receive the necessary support to allow us to do so. Our hospice team provides support to ensure the highest quality of life possible for the person living with a life-limiting illness.
Considered to be the model for quality, compassionate care for people facing a life-limiting illness or injury, hospice and palliative care involve a team-oriented approach to expert medical care, pain management, and emotional and spiritual support expressly tailored to the patient’s needs and wishes. Support is provided to the patient’s loved ones as well.
Hospice services are available to patients of any age, religion, race, or illness. Hospice care is covered under Medicare, Medicaid, most private insurance plans, HMOs, and other managed care organizations.Paying for Hospice
Hospice & Palliative Care is a family-centred approach that includes, at a minimum, a team of doctors, nurses, home health aides, social workers, chaplains, counselors, and trained volunteers. They work together focusing on the passing person’s needs; physical, emotional, or spiritual.Our Care Team
Hospice focuses on caring, not curing, and in most cases care is provided in the patient’s home. Hospice care can also is provided in free standing hospice centres, hospitals, nursing homes and other long-term care facilities.

CHAP accreditation demonstrates that Soreo Hospice & Palliative Care meets the industry’s highest nationally recognized standards. Rigorous evaluation by CHAP focuses on structure and function, quality of services and products, human and financial resources, and long term viability.

CHAP is an independent, not-for-profit, accrediting body for community-based care organizations. Created in 1965, CHAP was the first to recognize the need and value for accreditation in community-based care. CHAP is the oldest national, community-based accrediting body with more than 5,000 agencies currently accredited nationwide.
Through “deeming authority” granted by the Centers for Medicare and Medicaid Services (CMS), CHAP has the regulatory authority to survey agencies providing in-home support, hospice, and home medical equipment services, to determine if they meet the Medicare Conditions of Participation and CMS Quality Standards. CHAP’s purpose is to define and advance the highest standards of community-based care.
